Information form for firm and owners/directors enrollment.
Must be signed by an owner/director and completely filled in.
Corporate Documents
Must be filed with and stamped by the relevant body (Board of Trade, Notary,
etc.).
Firm Type - Ltd Liability.
Bylaws in case of first enrollment
Bylaws amendment in case of capital increase (where applicable)
Bylaws amendment in case of admission/exit of partners (where applicable)
Firm Type - Corporation
Minutes of the shareholders meeting that approved the statutes in force -
in case of first enrollment
Minutes of the latest capital increase
Minutes of the election of the current Board of Directors
Minutes of the election of the current Board of Administration (where
applicable)
Accounting Statements
Must include the firm's corporate name, final date, signature of the
manager and accountant, with the latter's name and registration No. (CRC). In
case of first enrollment, send accounting statements for the past three
corporate periods or the past two corporate periods and the latest interim
balance-sheet.
Balance Sheet/Interim Balance Sheet and P&L Statement
Exempted firms or those who chose the assumed profits system
must attach a copy of their income tax return.
